Four ms register of cases, completed by Betty Turner during her time practicing as a midwife at the City Hospital in Plymouth and then at the City Hospital in Nottingham (5 July 1946-24 March 1954), containing the name, ages, address and pregnancy history of each patient that she attended in childbirth, the presentation of the child at birth, the date and hour of the child’s birth, the sex of the child and whether the child was born living or dead, whether the child was premature, the name of the doctor if called, if there were any complications during labour, the date of her last visit, the condition of the mother and child then, as well as any other further remarks, such as the drugs administered or whether forceps were used during the delivery, found together with a photograph of three midwives in uniform, one of which is identified on the back as Emily Dear (circa. 1940-1950). The first register (5 July 1946-24 July 1948), appears to record the details of the women she attended who experienced complications during labour who went on the have forceps deliveries or caesarean sections.
