Completed performance appraisal forms. These give the job holder a rating from 1 (outstanding) to 6 (unsatisfactory) followed by a written explanation which describes the employee's performance and quality of work over the past year, gives areas which need improvement, and details growth potential and development and training plans. These are sometimes accompanied by Development and Training Record sheets which give details of education, membership of professional bodies, work experience before and after joining the company, short courses and conferences attended, and other development and training activities.
