Includes details of date of admission; name; age on admission; by whose order detained and date authority expires; result of examination of admission; signature of medical officer and the hospital and date; date of discharge or death; result of examination on discharge; method of disposal either admitted to the hospital for psychiatric treatment as a voluntary, temporary or certified patient, admitted to another hospital, dealt with under the mental deficiency acts, to the care of friends or own home, or otherwise disposed of.
